Can Copywrited Templates be scaned and reproduced?

I have a pad that has a format that I would like to reproduce electronically. The page has a copywrite icon on it. Is it possible to scan this template into my Mac, for use as an index to my journal?

Possible, yes.
Legal, most likely not, unless you are or have permission from the copyright holder.
The actual legal case and/or attitude of the copyright holder probably depends on the use to which you intend to put the copyrighted work.
